About the Bradford Reablement Service

Bradford Council’s Reablement Service supports people to regain their independence, confidence and daily living skills following illness, a hospital stay, or a change in circumstances.

Our aim is to help you do as much as possible for yourself and live safely and independently at home.

What is Reablement?

  • Reablement is a short-term, goal-focused service delivered in your home
  • It supports you to rebuild skills, confidence and independence
  • The focus is on helping you learn or re-learn how to manage daily activities yourself, rather than doing things for you

How we support You

  • We will work with you to agree goals that matter to you.
  • You will be supported to practise daily activities such as:
    • Washing, dressing
    • Moving around your home
    • Preparing meals
  • Your support will be regularly reviewed to help you make progress towards independence

Who can access the Service?

Reablement is for people who:

  • have had a recent illness, hospital stay or change in circumstance.
  • are experiencing difficulties with daily activities
  • Have the potential to regain or improve their independence with support at home

Reablement may not be suitable if you need long-term ongoing care or support.
The professional supporting you may discuss with you whether reablement is the right service for your needs before any referral is made.

How to access Reablement

  • Referrals are usually made by hospital teams, social care staff, or health professionals.
  • If you think reablement could help, you can contact Adult Social Care who will advise whether reablement is a suitable service for you

What happens at the end of reablement

  • Reablement is time-limited and focused on helping you become as independent as possible
  • Many people do not need ongoing support after reablement
  • If needed, we will discuss:
    • Advice and guidance
    • Equipment or adaptations
    • Longer-term support options
