What information do we collect

We may collect some or all of the following information about you depending on which of our services you use:

Common to all services

• Name, age, date of birth, ethnicity
• Correspondence details (address, telephone number, email addresses)
• NHS number, medical conditions

Public Health Intelligence

• Births records – including details of a child’s birth including date of birth, NHS number, ethnicity, postcode
• Deaths records – including details of a person’s death, what they died of, where  they lived, GP they were registered
• Data from the National Child Measurement Programme including Weight and height of children, school attended

Stop Smoking Service

• Details of pregnancy

Environmental Health

• Details of enforcement / investigation activities
• Recordings of formal interviews

Health Improvement

• Weight, height, medical conditions

Why do we use this information?

We use this information to:

• Monitor the health protection and infection prevention issues within the district
• Ensure that health, social care and public health services address local health needs and are focused on reducing health inequalities (unjustifiable differences in health status) in relation to birth rates, low birth weights and still birth between groups and areas.
• Assist individuals to improve their general health and wellbeing through a variety of different stop smoking services
• Assist in the administration of Environmental Health enforcement and investigation activities.
• Monitor service use through performance management of contracts and analysis of data to assess need in the district
• Assist individuals to improve their general health and wellbeing through a variety of different health improvement programmes
• Produce Health Needs Assessments eg Joint Strategic Needs Assessment, Pharmaceutical Needs Assessment
• Protect or improving public health
• Measure the health, mortality or care needs of the population eg health and wellbeing surveys
• Undertake Public Health planning, monitoring and evaluation
• Commission and deliver services to promote health and prevent ill health eg sexual health services, diet and weight management, sports services, stop smoking services, NHS health checks

What authority does Bradford Council have to collect and use this information?

Legal obligation under various UK laws including but not limited to:

• The Health & Social Care Act 2012 (plus consent under common law duty of confidence)
• The Statistics and Registration Service Act (2007) as amended by Section 287 of the Health and Social Care Act (2012)
• Regulation 3 of the Health Service (Control of Patient Information) Regulations 2002.)
• The Equality Act 2010
• Contract (active sports, grants) Consent (surveys, consultations diets, stop smoking, sexual health etc.)

Who are we likely to share this information with?

We may sometimes share the information we have collected about you where it is necessary, lawful and fair to do so. In each case we will only share the minimum amount of information, only when required, for the following reasons:

With other government departments including the NHS to:

• Notify the NHS of any health protection issues
• Update relevant NHS records relating to the individual

The police and other crime agencies to:

• Provide information on how the relevant services are being accessed and why

With the Born in Bradford Study to:

• Support research undertaken (with consent or where the law allows it).  A data sharing agreement is currently in place to facilitate this

Coronavirus (COVID-19)

The Secretary of State for Health & Social Care has given notice under Regulation 3(4) of the Health Service Control of Patient Information Regulations 2002 that the Council is required to process and share necessary health and social care data for the COVID-19 pandemic. This will involve the processing of personal data (such as name, address, workplace, travel details, etc.) of individuals, including in many cases sensitive, ‘special category’ personal data such as data relating to health information.

Data protection law does not stand in the way of the provision of healthcare and the management of public health issues; therefore we take important considerations into account  specifically for Coronavirus COVID-19 when handling personal data in these circumstances , particularly health and other sensitive data. These measures will be proportionate and processed for specific purposes only

How do we keep this information secure?

Your information is stored securely on databases and document management systems with stringent access and use policies. We also undertake quality checks and monitoring to ensure the information we hold is accurate at the time and being used appropriately.

How long do we keep this information?

We have a record retention schedule which details how long we keep different types of records and what we do with them when we no longer need them.
