What is the Bradford Learning Disabilities Transformation Plan?

Bradford Learning Disabilities Transformation Plan is an all age change programme focusing on improving services for people with learning disabilities who may have autism, who display behaviour that challenges, including those with a mental health condition. This programme will drive system-wide change and enable more people with a learning disability who have complex behaviour presentations, to have a home within their community, be able to develop and maintain relationships and get the support they need to live healthy, safe and rewarding lives.

Bradford Airedale and Craven’s local plan is developed with involvement from the Learning Disabilities Partnership Board and Autism Partnership Board and links to existing structures. Integrated personalisation, co-production, engagement and communication with people with learning disabilities and/or autism and family carers will be embedded across the programme.

Bradford Transforming Care Programme - Provider and partner events

This work is aimed at specialist providers and partners who can support people with complex needs and presentations, including those with challenging behaviour and those who form part of the Transforming Care Programme cohorts.
