MASCOT Children's Team Multi Agency Specialist Children's Team serving Bradford & Airedale Districts.
Our aim is to offer district wide support and advice to children with life limiting conditions, and their families, in all aspects of palliative care through life, illness, death and bereavement, irrespective of culture, beliefs and faith. In addition, using a coordinated approach, we aim to support the professionals involved with these families with specialist advice and programmes of education.
The service offers:
- Specialist advice around symptom management and end of life care.
- To identify and develop services through strategic engagement of key agencies.
- Support to the child and family from diagnosis through illness, death and bereavement.
- To provide formal/informal programmes of education across the wider organisations in aspects of paediatric palliative care.
- Act as a resource to professionals involved with children with a life limiting condition, and their families.
- To provide direct care to the child and family e.g. physiotherapy, sibling support.
- Close liaison with services to meet the needs of this group of children.
- Additionally, medical support can be sought from local paediatricians and the Paediatric Palliative Care Consultant for the Yorkshire region.
Referral:
Any child/young person with a life limiting/life threatening condition aged 0 - 19 years.
With parental consent, all referrals are discussed at a weekly allocation meeting and allocated appropriately.
