Continuing Healthcare Training
During this half day course delegates will learn about the process for CHC in the Bradford District, and who is who through the process; they will complete a CHC screening checklist, and will work in a simulated multi-disciplinary team through a Decision Support Tool in order to make a recommendations for continuing care funding or NHS Funded nursing care. Delegates will work closely with colleagues from other sectors, helping to promote understanding across different areas of involvement in the assessment process.
Who the course is for:
Adult Social Work or CCO's who come into contact with people who may be eligible for continuing care funding.
