How do they help?
We will produce a care support plan with you. This will show your strengths and skills and also where you need support.
How can I apply?
Usually, you can contact your chosen day centre directly to talk about your interests.
We will usually visit you at home to talk about your options.
If your needs are more specialist or complex we will carry out an assessment at your home.
What is an assessment?
In an assessment we gather information to work out what difficulties you, or your carer, are having and what sort of help you are asking for, then we work out what help we can offer
What happens after the assessment?
We will take all your needs into careful consideration and speak to you about the best option for you from a range of options.
Is there a charge?
You will need to contribute to the cost of attending day care or a day centre / luncheon club. This depends on how often you attend and your personal circumstances.
Your ability to pay will be assessed and your social worker will tell you about this.
Most grant-funded voluntary and community groups make a small charge towards food, refreshments and transport.
Can I get help with the cost?
After your assessment we will be able to tell you if you can get help with any costs.
It’s my first time, who will meet me there?
On your first visit a member of staff who will show you around the centre. You will be able to talk other people like yourself. You will have plenty of opportunity to ask any questions before you make a decision. We will give you the contact details of the person(s) meeting you on the day prior to your visit.

I want more choice and control
If you want to arrange services and support for yourself, then you might be interested in Self Directed Support.
This gives you an Individual Account or Direct Payment from us which you can use to arrange your own support.
You can look after the money yourself or ask the Council to hold the money and help you find the support you need.
You can discuss this at the time of your assessment with your social worker.
