- Please note that this form needs to be printed out double-sided. If you are not able to print double-sided, please contact the Electoral Services Unit.
- If you are unable to attend a Polling Station either because you have a physical incapacity or because of your occupation or employment or are on holiday, you may appoint another person to vote on your behalf. This is known as a proxy vote. If you wish to apply for a proxy vote you can use the form below.
Application Form to Vote by Postal Proxy
- Your proxy may wish to vote by post if they are unable to attend the Polling Station to vote in person. If you wish to apply for a postal proxy, please contact the Electoral Services Unit.
